I follow a very basic diet plan which means that I dont need to monitor what I eat too much, I eat three meals a day within 8 hours, loads of meat, vegetables, eggs milk and nuts, protein powder as well.

So for me It's not necessary to measure what I eat, but for some people it might be.

I find it too stressfull if im recording absolutely everything I do, so I only record the important stuff, the big lifts and big runs if I do them.

When I'm in the gym I record my power sets only for my main lifts in a logbook. Deadlifts, Squat, Bench, Dips, Chin-ups. I don't record warm-up sets or secondary exercises or stretching.

When I ran I tracked my pace and distance with Nike+ for the Iphone, although you can take readings off of a treadmill in you run on one.

I also record my times for completion of circuits if I am using them in my training.
